Our Basic Privacy Policy

Your privacy is important to you. It’s important to us too. That’s why we won't share your information with anyone unless we get your permission first, and we will never sell your information.

We only collect private information that you choose to share with us. For example, information that you provide when you sign up for the Alzheimer's Prevention Registry or when you request details about participating in an Alzheimer's prevention study.

You can remove yourself from the Registry and unsubscribe from our emails at any time.

The Alzheimer’s Prevention Registry is managed by Banner Health. Banner Health complies with all local, state, and federal laws when it comes to patient privacy. The Long Version

Banner Health's Alzheimer's Prevention Registry ("APR") is committed to protecting your privacy. The APR website (www.endalznow.org) is designed so that visitors (“Users”) can visit without identifying themselves or revealing any personal information. If you choose, you may create a secure account, and register to receive communications from APR, including newsletters on new developments in Alzheimer's research and emails about research opportunities. In order to register, you must be 18 years and older and must provide a unique email address.

You can decide what information you choose to share with APR. You may choose to provide personally identifiable information including an email address or a telephone number which APR can use to identify or contact you. You may delete your personal information or withdraw as a registered APR member at any time. All you have to do to remove your information is to follow the "opt-out" link at the bottom of the emails APR sends you. Any information you give APR will remain confidential and will not be shared with any third parties, unless you specifically authorize APR to share your information.

Some information is automatically received and sometimes collected from you when you visit the website. We also use cookies to better understand how users use this website. Our cookies contain no Personal Information and are neither shared nor revealed to other websites. APR does not look for or at other websites' cookies on your computer.

APR collects Personal information only if you voluntarily submit it. On some pages, Users and registered members can update contact information; join APR (by providing APR with an email, first name, last name, gender, race, zip code, and area of Alzheimer research interest), or indicate interest in participating in research studies . The information collected by APR is not Personal Health Information as defined under the U.S. Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and its implementing regulations. However, APR will use similar patient privacy laws as a guide in protecting Registered Member's Personal Information. APR recommends that you do not share personal medical information through this website. APR will use personal information only for the purposes for which the information was submitted.

B. Use of Collected Information

Generally, the personal information you provide to APR will only be used for the purpose for which it was collected. APR will send email alerts about Alzheimer's research opportunities, resources to help fight Alzheimer's and ways to connect communities of people to advance Alzheimer's research, prevention and cure. When it comes to participating in research opportunities, you must "opt-in" in order for APR to share your personal information with any of APR’s research partners. Additionally, some research opportunities might require you to complete an online consent form in order to proceed with the research opportunity.

The personal information that you provide may also be used by APR in a de-identified form, i.e. a form through which individuals are not identifiable and cannot be contacted, for APR planning and administrative activities. Any information that you provide may also be collected for statistical purposes. APR may perform analyses on user behavior from time to time to measure user interest in various areas of this website.

APR may also be required by law to disclose information, including personal information. Although APR makes every effort to preserve your privacy, APR hereby notifies all users that APR will disclose information collected on the website if required by law to do so.

Consent to Disclosure: By providing your personal information to APR, you understand and consent to the collection, processing, transfer and use of information for the purposes set forth by APR as described under this Policy.

D. "Opt-Out"/ Correcting/ Updating/Deleting/Deactivating Personal Information

If you have provided information to APR and decide that you no longer desire to use this website, you may "opt-out", delete or deactivate your account by following the "opt-out" links at the bottom of the emails that APR sends.

You may also update or correct your personal information by emailing us at [email protected]. The information you provide will not be sold or given to any third party.

E. Updates to Privacy Policy

APR may in its sole discretion, update this Policy at any time and will post any amended Policy on this website. THE AMENDED POLICY SHALL BE EFFECTIVE UPON POSTING AND ACCESS TO OR USE OF THIS WEBSITE FOLLOWING AN UPDATE SHALL BE DEEMED CONSENT TO THE REVISED POLICY.
