Banner Health's Alzheimer's Prevention Registry ("APR") is aware of privacy concerns that visitors ("Users") and Registered Members might have, and it is committed to protecting your privacy. APR has designed its website (www.endalznow.org) so that Users can visit without identifying themselves or revealing any personal information. If Users of APR choose to receive communications such as newsletters about the state of Alzheimer's research, and emails about research opportunities, then Users can become Registered Members by creating a secure account and registering with APR. In order to register, the User must be 18 years and older and must provide a unique email address (that might belong to an individual or multiple individuals in the same household).
Additionally, the Registered Member may choose to provide personally identifiable information ("Personal Information"). Personal Information is information which APR can use to identify or contact you including Registered Member's name and email address. A Registered Member may delete their Personal Information from APR or withdraw as a Registered Member from APR at any time by following the "opt-out" link at the bottom of the emails APR sends you. Any information given to APR by its Users and Registered Members is not shared with any third parties and is kept confidential. The information will only be used to support the User's relationship with APR.

a. Automatically Collected Information
Some information is automatically received and sometimes collected from you when you visit the website. APR collects Personal information only if Users and Registered Members voluntarily submit it. On some pages, Users and Registered Members can update their contact information; join APR (by providing APR with their email, first name, last name, gender, race, zip code, and area of Alzheimer research interest). The information collected by APR is not Personal Health Information as defined under the U.S. Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and its implementing regulations. However, APR will use similar patient privacy laws as a guide in protecting Registered Member's Personal Information.
Furthermore, APR recommends that Registered Members not share personal medical information through this website.
APR will use Personal Information only for the purposes for which the information was submitted.
B. Use of Collected Information
Generally, Registered Member Personal Information will be used for the purpose for which it was collected. APR will send email alerts about Alzheimer's research opportunities, resources to help fight Alzheimer's and ways to connect communities of people to advance Alzheimer's research, prevention and cure. For each research opportunity communicated by APR to a Registered Member, Registered Member must "opt-in" for APR to share Registered Member's Personal Information with research partner. Furthermore, some research opportunities might require Registered Members to complete an online consent form to proceed with the research opportunity.
Registered Member information may also be used by APR in a de-identified form, i.e. a form through which individuals are not identifiable and cannot be contacted, for APR planning and administrative activities.
Users and Registered Members information may also be collected for statistical purposes. APR may perform analyses on user behavior from time to time to measure user interest in various areas of this website.
APR may be required by law to disclose information, including Personal Information. Although APR makes every effort to preserve every User and Registered Member's privacy, APR hereby notifies all users that APR will disclose Personal Information and other information collected on the website if required by law to do so.

D. "Opt-Out"/ Correcting/ Updating/Deleting/Deactivating Personal Information
If a Registered Member's Personal Information no longer desires to use this website, then the Registered Member may "opt-out", delete or deactivate their account and relinquish their status as a Registered Member of APR by following the "opt-out" links at the bottom of the emails that APR sends.
Personal Information may be updated / corrected by Registered Members when necessary. Registered Member may update their Personal Information here: (insert link for updating information) Updated information will automatically be updated in APR's database. The information provided will not be sold or given to any third party.
APR may in its sole discretion, update this Policy at any time and will post any amended Policy on this website. THE AMENDED POLICY SHALL BE EFFECTIVE UPON POSTING AND USERS AND REGISTERED MEMBER'S ACCESS TO OR USE OF THIS WEBSITE FOLLOWING AN UPDATE SHALL BE DEEMED CONSENT TO THE REVISED POLICY. APR encourages every User and Registered Member to visit this page regularly and especially prior to providing or updating any Personal Information.
